CARE Chest provides free medical resources to individuals who qualify at 138% of the Federal Poverty Level. A team member can guide you through our application process and potentially arrange delivery if you are homebound. You may arrange to pick up your monthly supplies through our Mobile Outreach locations or at our main warehouse locations in Reno or Las Vegas

During the application process, in order to pick up your supplies, please upload proof of Nevada residency, identity, and income. We may also require a prescription for supplies based on which services you are applying for.

Please be ready to provide a current proof of income, current proof of address, and driver’s license or state-issued ID.
For first-time consumers to qualify, and annually thereafter, please provide the following documentation:
- Proof of identity (a driver's license or state-issued I.D.)
- Current proof of residence (a lease, power bill, bank statement, etc)
- Current proof of income (pay stubs, direct deposit slip, social security documents or tax document)

Durable Medical Equipment & Supplies
Durable medical equipment is loaned for as long as it is needed, consumable items such as incontinence supplies and non-prescription items can be re-ordered monthly for consumers who qualify. If a consumer does not qualify to receive Durable Medical Equipment because monthly income is too high, they may enroll in Club CARE Chest for an annual $50 membership.

Prescription Assistance
CARE Chest provides up to $300 per client annually for CARE Chest-approved prescriptions (eg., cardiac, diabetic, antibiotic, pulmonary, or psychotropic medication).
Some people choose to use our prescription assistance to offset the cost of co-pay.
CARE Chest does not cover the cost of pain or cancer medications.

Diabetic Supplies
CARE Chest’s Diabetic Supplies program distributes testing supplies, including blood glucose monitors, test strips, syringes, and lancets. Supplies are available monthly with a prescription.

Supplemental Medical Nutrition
CARE Chest provides doctor-prescribed supplemental medical nutrition such as Ensure, Glucerna, Pediasure, and Instant Breakfast to consumers on a monthly basis.
